Bictegravir/ Emtricitabine/Tenofovir alafenamide (BIC/FTC/TAF)
Rifabutin
Quality of Evidence: Moderate
Summary:
Coadministration with moderate inducers, such as rifabutin, is not recommended in the product labels for Biktarvy (bictegravir/emtricitabine/tenofovir alafenamide). Coadministration of rifabutin (300 mg once daily) and bictegravir alone (75 mg once daily) decreased bictegravir Cmax, AUC and Cmin by 20%, 38% and 56%, respectively. In a study with 19 people with HIV on rifabutin-containing therapy and Biktarvy for at least 28 days, bictegravir Cmin decreased by 86% (median concentrations of 438.3 ng/mL (IQR 400-781) vs 3261 ng/mL (IQR 1957-4696)); one participant had a bictegravir Cmin of 144.2 ng/ml. One-year viral suppression was achieved in all participants despite the significant interaction with rifabutin. In a PBPK modelling study, metamizole (another moderate inducer) was predicted to reduce bictegravir Cmax, AUC, and Cmin by 23%, 38%, and 56%, respectively. Predicted median and 5th-percentile bictegravir Cmin values were 1309 and 554 ng/mL, respectively, which were approximately 8- and 3-fold above the protein-adjusted EC95 (162 ng/mL), respectively. For comparison, median bictegravir Cmin concentrations during the 2nd and 3rd trimesters in pregnant women who were virologically suppressed on once daily Biktarvy were 61-67% lower than postpartum and the American product label for Biktarvy advises Biktarvy can continue to be administered during pregnancy to individuals who are virologically-suppressed in the absence of any known drug resistance mutations. These observed drug interaction data and PBPK simulations suggest once daily Biktarvy would still achieve bictegravir Cmin concentrations above the efficacy target. Coadministration of once daily Biktarvy with rifabutin could be considered in selected individuals who are virologically suppressed and adherent to therapy and who have no known resistance-associated substitutions to any of the individual components of Biktarvy. However, this combination is not recommended in individuals with factors that could further reduce bictegravir exposure (e.g., overweight/obesity, pregnancy, or concomitant use of mineral supplements) or in those unable to take Biktarvy at regular intervals (every 24 hours).
