Raltegravir (RAL)
St John's Wort
Quality of Evidence: Very Low
Summary:
Coadministration has not been studied. The European product label for raltegravir states that St John's wort may be used with twice daily or once daily raltegravir at the recommended doses. However, St John's wort is a PXR agonist and therefore could potentially induce UGT1A1 and reduce raltegravir exposure. Monitor response to antiviral therapy. A recent study suggests a low risk of a clinically relevant pharmacokinetic interaction with low-hyperforin formulations (<1 mg/day) of St John’s Wort (hyperforin is the constituent responsible for induction of CYPs and P-gp).
